One guide. Every step.

Your family is facing one of the biggest decisions you'll ever make — and you don't have to face it alone.

Most families juggle a realtor, a facility, and a home care agency — separately, and tell the same story to each one. Senior Transition Guides is one trusted guide who sees the whole picture: the care, the home, and the money.

Serving Outagamie, Winnebago, Brown, Shawano & Oconto counties, Wisconsin.

Why families call Julie first

I've stood where you're standing.

I know what it feels like to be the one who doesn't know what comes next — I lost the grandparents who raised me, with no guide of my own to turn to. That's why I became the guide I needed and never had.

I bring the clinical knowledge of a nurse, the leverage of a licensed real estate specialist, and the compassion of someone who has known deep loss. No other guide in the region can see the full picture — medical needs, housing, and finances — under one trusted name.

"I came from nothing and built something — not to prove a point, but to honor the people who believed in me when they had no reason to. Now I use everything I know to guide seniors and their families through one of the most important transitions of their lives. That's not just a business. That's a calling."

Serenity Gardens

Assisted Living — 19 Units

Nursing-led residential care when daily support is needed.

🤝

Mano Po Home Care

Home Care Agency

Non-medical in-home support for families who want to age in place.

Each service refers into the others as your needs change — a home sale can lead to assisted living or home care, and home care can transition into Serenity Gardens if more support is ever needed. One credential sits on top of all three, so you only tell your story once.

How It Works

Three steps. One guide. No guessing.

You don't need a plan for every possible scenario. You need the next right step — and someone who already knows your story to walk it with you.

01

Start the conversation

One call. Tell your story once — not to five different offices. Reach Julie directly at 920.850.6175.

02

Get a clear picture

Julie walks through your family's specific situation — care needs, timeline, home, and budget — and lays out the real options in plain language.

03

Move forward with a plan

Whichever path fits — assisted living, home care, or selling the house first — you'll know exactly what happens next, and who to call.

What waiting costs — and what a guide changes

Families rarely regret calling too early. They regret calling too late.

Without a guide

  • A fall or hospital discharge forces a rushed, crisis-mode decision
  • You call facility after facility — some never call back
  • You manage medical, housing, and financial decisions alone, on top of your own family and job
  • Mom or Dad ends up somewhere that feels big, impersonal, and unfamiliar

With Senior Transition Guides

  • One trusted person already knows your story when you call
  • You get a clinical, financial, and housing picture in one conversation
  • Your parent is cared for with the dignity of the Mano Po tradition
  • You get to be a daughter or son again — not a full-time care coordinator

Why This Work Matters To Me

The story behind Nourish the Future.

I grew up in the mountains of the Philippines — no electricity, no running water, no vehicles. Only the faith my grandparents poured into me every single day. When I lost them at twelve, I was on my own — working as a maid before and after school in exchange for food and shelter, waking at 4am and wearing borrowed shoes with holes in them. What kept me going was one stubborn conviction: sacrifice now, and enjoy the fruit of your labor the rest of your life.

I arrived in the United States in 2007 and never looked back. I built a nursing career, a real estate career, and eventually Serenity Gardens. But I never forgot the children I left behind — the ones still being told, the way I once was, that where you start is where you'll stay. So I started Nourish the Future Inc., a nonprofit that feeds hungry children in the Philippines, sponsors their education, and teaches them what almost no one taught me until much later in life: that a good life is every child's birthright, and born poor does not mean destined poor.

It's still small — funded mostly out of my own pocket today — but it's growing. Ten percent of Serenity Gardens' profits go directly to Nourish the Future, because every part of what I've built is meant to circle back to the people who need it most.

What Nourish the Future does

  • Feeding Program — hot, nutritious meals for children in poverty in rural Philippine communities and schools
  • Educational Sponsorships — tuition, food, and lodging support for high school and college students
  • Dream Workshops — teaching children to imagine and plan a life beyond survival
  • Orphanage, Phase 1 — building toward a safe, permanent home for 20 children
